Title of Job: ASSESSMENT AND ENROLLMENT COORDINATOR

The Family Center is implementing a new data base, EVOLV-CS, to track all outreach and service activities as well as report outcomes. The Assessment and Enrollment Coordinator is responsible for both completing initial intake assessments and follow-up to ensure that new clients are connected to services effectively and efficiently and for ensuring that all assessment and outcome data going into EVOLV-CS, is accurate and timely. The Assessment and Workflow Coordinator reports to the Director of Social Services and works closely with other Department heads and supervisors.
Responsibilities:
- Conduct intake assessments for all new clients by phone and triage based on needs and urgency.
- Provide referrals and information to callers not eligible for The Family Center’s services.
- Participate in weekly case assignment conferences to review eligibility determinations and assignment of clients.
- Ensure that all new clients are thoroughly assessed and expediently assigned to appropriate programs and staff.
- Set and monitor workflow tasks related to case assignment, assessment, service planning and outcome measurement using database monitoring tools and reports.
- Monitor information entered into referral organization database component to ensure non-duplicative, accurate records for all community partners.
- Work closely with managers and supervisors to maximize integration of all database functions into work of all staff, pinpointing areas for additional support, and training.
- Some field work required.
- Special projects as requested.
Requirements:
MINIMUM QUALIFICATIONS:
- Bachelors Degree required, Masters Degree preferred.
- At least two years experience in social/medical/public health research or service environment.
- Excellent assessment and communications skills.
- Excellent computer skills and experience with database systems.
- Bilingual Spanish required.

The ESLS Outreach Specialist oversees outreach for a new program which focuses on early childhood development of children who are HIV-affected and under 7 years old. The ESLS Outreach Specialist reports to the Program Coordinator.

RESPONSIBILITIES
- Coordinate all program outreach efforts and activities.
- Assist Program Coordinator in developing and implementing targeted outreach strategies to meet program deliverables.
- Establish and maintain relationships and formal linkages with community-partners, including CBOs, medical providers, government offices, professional coalitions and community business.
- Coordinate and provide trainings and presentations on various aspects of The Family Center’s services to a range of audiences.
- Conduct community visits including home visits
- Maintain a small caseload
- Conduct intake assessments and new client orientation.
- Assess needs and provide information, advocacy and referrals to meet those needs.
- Coordinate and deliver group interventions, including support groups, recreational programs and educational workshops.
- Oversee planning and operations for Saturday programming, including managing the calendar of Saturdays activities, addressing facility, personnel and client-related issues that arise.

- Bachelors degree in Social Work or related field
- Tuesday thru Saturday schedule

Title of Job: Legal Intern
Description: The Family Center’s Department of Legal Services assists clients in a number of practice areas including: (1) Trust and Estates (standby guardianships, wills, advanced directives); (2) Family Law (matrimonial work, support, custody, and guardianship for children and adults); (3) Income maintenance (fair hearings, SSI/SSD); and (4) Housing (nonpayment, holdover, repairs cases and succession planning). We work with clients on both transactional and litigated matters and meet with clients in our offices, their homes, hospitals and local CBO’s.
For Interns, we provide a full orientation in our areas of practice and make sure they have exposure to a wide range of issues and courts

Staff Attorneys practice in one of three distinct substantive areas including: (1) Family Law; (2) Public Benefits Law; and (3) Housing Law. Additionally, attorneys provide lifetime planning services for clients in the form of wills and advanced directives. For attorneys, we provide a full orientation in our areas of practice. TFC seeks an attorney to work full time in the area of public benefits. The Staff Attorney reports to the Director of Legal Services.
Responsibilities:
- Interview clients as to legal matters at least once per week over the phone and in person
- Represent clients in court, administrative agencies, and other forums in Family Law or Housing Law or Public Benefits Law matters
- Conduct outreach activities and clinics off-site
- Mentor Volunteer Attorneys through TFC’s Pro Bono Program
- Maintain proper files and aid staff in the preparation of research, funding reports and applications
- Keep current on law and procedures within their area of practice.
Requirements:
- Juris Doctor and a demonstrated interest in family, public benefits or housing law

Title of Job: YouthNet Case Planner
Description: YouthNet is funded by the NYC Administration for Children’s Services to address the needs of teens coming out of congregate care or at risk of out-of-home placement, by providing a rich menu of intensive home and office-based services including individual and family therapy and conflict resolution. Youth Net operates out of the agency’s Brooklyn office in the Bedford-Stuyvesant neighborhood. The case planner will provide services to youth and their families utilizing motivational interviewing and conflict resolution (collaborative problem solving) techniques.
The YouthNet Case Planner reports to the clinical supervisor.
This is an excellent opportunity for a clinician to provide services to underserved high risk adolescent and their families in their natural environment. You will be able to first hand experience the concerns of the families we serve. It will prove to be an invaluable experience in the development of your career as a social worker.
Responsibilities:
• Provide home- and office-based therapeutic services to high-risk teens and their families residing in Brooklyn, including individual, family and group work.
• Utilize Motivational Interviewing and Collaborative Problem Solving techniques to help families to meet their goals.
• Complete comprehensive assessments and bio-psycho-socials.
• Complete Progress Notes, Family Assessment Service Plans (FASPs) and other documentation as required by the funding agency and program.
• Participate in weekly clinical supervision, unit meetings, and family team conferences.
• Develop and facilitate groups for teens, parents/caregivers and families.

Requirements:
• Masters in human services discipline such as social work, psychology, art therapy or counseling.
