New User Request Forms
Please click on a link below and fill out the new user request form for your new hire. The database administrator will use this form to set them up as a user in the appropriate database. They will then send an email with the username, password, and link to the database to the new hire and cc you. They will also send some basic instructional videos on how to use the database, however, it is up to you to train your staff on the specifics of how you want them to use the database for your program/department.
